Occupational therapy services across Melbourne’s north

If you are looking for occupational therapy in Epping, Wollert or Lalor, the starting point is understanding what everyday activities are becoming difficult and why. Cas Health works with adults to assess function in the places where daily life happens, then provides practical recommendations linked to the person’s goals, safety and independence.

Depending on the referral and clinical scope, services may include:

  • Functional Capacity Assessments for daily living, participation and support needs
  • Home and Living Assessments where a person’s living situation or support model needs review
  • Assistive technology, including equipment trials, recommendations and reports where appropriate
  • Home modifications to improve access, safety and independence
  • Seating, mobility and pressure care assessments
  • Home safety, falls prevention and daily living assessments
  • Practical strategies, education and collaboration with the person’s support team

Why home visits help

Seeing the person in their usual environment can show how access, furniture, equipment, routines and support arrangements affect daily function. It also helps the occupational therapist make recommendations that fit the actual home rather than an unfamiliar clinic room.

Who Cas Health supports

Cas Health provides adult occupational therapy for people aged 18 and over. Referrals may relate to disability, neurological or musculoskeletal conditions, injury, ageing, changes in mobility, falls, fatigue, pain, cognition, sensory needs or difficulty managing everyday activities.

A client, family member, support coordinator, care manager, case manager, GP or another health professional can make an enquiry. Each referral is reviewed for clinical suitability, location, required timeframes and current therapist capacity.

Funding options for occupational therapy

The funding pathway affects service agreements, approvals, travel and reporting requirements. Cas Health confirms these details before the first appointment.

What to expect from the Cas Health OT process

Every referral is different, but our occupational therapy process generally follows these six steps. We will explain what applies to your situation before services begin.

Cas Health Occupational Therapy referral process, from initial contact and assessment to goal setting, intervention, reports and ongoing support in Melbourne.

1. Referral or enquiry

Submit our online referral form, call, email or enquire through the Cas Health website. Providing the client’s suburb, funding type, reason for referral and any important timeframes helps our team review the enquiry efficiently. (Submit your referrals here: https://cashealth.com.au/referral-form/)

2. Initial Assessment

An occupational therapist meets with the client at home or another agreed setting to understand their goals, daily routines, functional needs and safety. Telehealth may be suitable for selected appointments, depending on the person’s needs and the purpose of the referral.

3. Goal Setting and OT Plan

Together, we identify the client’s strengths, challenges and priorities. The occupational therapist helps establish meaningful goals and outlines the recommended assessments, supports, equipment trials, reports and expected timeframes.

4. Intervention and Support

Support is tailored to the client’s goals and may include occupational therapy sessions, daily living or mobility skill development, assistive technology assessments and trials, home-modification assessments, practical strategies, education and training.

5. Reporting and Recommendations

Our occupational therapist prepares the documentation required for the referral. This may include a Functional Capacity Assessment, assistive technology recommendations, home-modification documentation, progress reports or letters of support. Recommendations are based on the assessment findings, functional needs and agreed goals.

6. Ongoing Review and Long-Term Support

When ongoing occupational therapy is required, we review progress and adjust goals or strategies as needs change. This may include follow-up training, equipment reviews, support for plan reassessments or renewals, and monitoring of the client’s long-term safety and independence.
