Home and Living Assessment
Home and Living assessments can help those with a higher level of support needs and more significant functional impairment with accessing supports and/or a home environment that is suitable to their unique needs and situation. These assessments are usually completed when an individual is in a living situation that is having a negative impact on their function and safety, and ability to participate in daily life. Home and Living assessments look at the individuals physical home environment and the support services in place and can provide recommendations as appropriate for:
- Accessible and safe housing via Specialise Disability Accommodation. This may include environments that enhance the function of those with sensory, physical, cognitive or behavioural difficulties.
- Increased and higher levels of in-home support via Supported Independence Living.
- More flexible and less structured support via Independent Living Options.
The aim of these assessments is ultimately to facilitate the individuals independence, safety and quality of life.
